Guatemala has made significant progress in improving the treatment of congenital heart disease through the introduction of advanced surgical techniques. In 1997, the United States initiated a program to transfer congenital heart surgery technology to Guatemala, leading to impressive outcomes. Congenital heart disease (CHD) refers to structural abnormalities in the heart that develop before birth, often due to genetic or environmental factors. These conditions are among the most common birth defects and can be life-threatening if not treated properly.
A study led by Larrazabal et al. used the RACHS-1 scoring system—a method designed to evaluate the risk and outcome of congenital heart surgeries—to analyze mortality rates from 1997 to 2004. The RACHS-1 classification takes into account the patient's age and the type of surgical procedure, dividing cases into six risk levels. A total of 1,215 patients were included in the study, with an average age of 3.1 years. The overall mortality rate was 10.7%, with 130 deaths recorded.
Importantly, the adjusted mortality rate showed a clear decline over time. It dropped from 10.0% between 1997 and 1999, to 7.8% between 2000 and 2002, and further to 5.7% between 2003 and 2004. This downward trend highlights the positive impact of the technological and medical support provided to Guatemala. It also demonstrates that risk-adjusted mortality statistics, such as those based on the RACHS-1 system, can effectively assess surgical outcomes even in developing countries where resources may be limited.
The success of this initiative not only improved survival rates but also strengthened local medical capabilities, offering hope for more children suffering from congenital heart conditions in the region. This case serves as a powerful example of how international collaboration can make a real difference in global health care.
